Sunset park Senior center has a bunch of services located upstairs from Rite Aid @ 53rd & 5th (enter up the ramp on 53rd street to the 3rd floor)  This location has been temporarily relocated to 43rd and 4th at Marien Heim Senior Center.

As a member of the National Alliance of HUD Tenants/Region 2 (NAHT) we work to support the cause of national efforts.  Annually we attend the Membership Conference that takes place in Washington, DC.  Please help us to fund our participation by making a donation OF ANY AMOUNT.

If you live in HUD Multi Family Project Based Section 8 and Mitchellama housing and want your Tenant Association to become a member, print and complete this FORM.  Attach your memberhsip dues of $25 and mail it in!. 

As a National Organization, we have a total of 10 Regions.  In order to apply your donation to the New York/New Jersey Region 2, go to the DONATION link on the home page of the National Alliance of HUD Tenantsand REMEMBER to add .02 to whatever your donation amount is!  (ie: $10.02).

It is best that your landlord use the HUD model lease when drafting your lease.  Please pay SPECIAL ATTENTION to ITEM #30 on PAGE 14.

RECERTIFICATION DOCUMENTS

ANNUAL RECERTIFICATION

BEFORE YOUR LEASE EXPIRES YOU SHOULD RECEIVE THE FOLLOWING:

120 Days BEFORE your annual recertification, you should receive your 1st reminder notice CLICK HERE

90 Days BEFORE your annual recertification, you should receive your 2nd reminder notice CLICK HERE AND

60 days BEFORE your required recertification appointment you should receive your 3rd reminder with an effective "termination warning"  CLICK HERE.


IF YOU FAIL TO RESPOND

You will receive a 30 DAY notice of SUBSIDY TERMINATION.  This means you will be responsible for the full amount of the rent with no subsidy.

 

DURING YOUR APPOINTMENT

DURING your Recertification appointment, Management is required to notify you of your annual recertification appointment deadline date as part of your lease CLICK HERE

 IN ORDER TO QUALIFY FOR A PROJECT BASED SECTION 8 SUBSIDY

Based on your family size as listed on your lease, your COMBINED income BEFORE TAXES AND DEDUCTIONS, cannot exceed a certain amount.  CLICK HERE to enter your logistical information.  

 DETERMINING INCOME & CALCULATING RENT 

HOW YOUR RENT IS DETERMINED  Is the HUD fact sheet for allowable deductions and assets counted toward your rent calulations.

If you want to know exactly how they calulate your rent it is a very lengthy reading (80 pages).  But if you want to have your rent calculated properly and/or you want to know how it is done. CLICK HERE 

 ELECTRONIC INCOME VERIFICATION PROCEDURES

As of January 2010, management companies are required to us the new EIV System.  This allows for full monitoring of your income from various sources, without you submitting it directly to them  Click Here to read the brochure. Management must adhere to these strict guidelines when assigning personal to screen your personal information.  Management cannot screen your income unless you sign this package CLICK HERE.  Refusal to sign this package can initiate termination of subsidy and possible eviction if your housing is dedicated to low income persons only.


ARREARS AND BILLING

REPAYMENT AGREEMENTS BETWEEN THE LANDLORD AND TENANT

Repayment agreements are REQUIRED when paying arrears of any kind.  This means you should be signing an actual agreement with terms that are outlined. You should make these payments in a separate check and not part of your monthly rent. Without this agreement there is no proof that you are paying toward your arrears or that unreputable management companies are even documenting these payments to HUD. CLICK HERE

HUD DOMESTIC VIOLENCE FORM In project based section 8 housing, if you are a victim of DV, you can request that 1. your abuser be removed from the lease and 2. in documented cases of violence, can request a transfer waiver, allowing you to move out of the unit. The same policy also gives the landlord the right to immediately evict the household if it is proven that your household DV situation can put other families at risk.  REQUEST FOR REPAIR FORM If you would like to document your request for repairs, print this form out and bring it to management. They must give you a copy if you ask for one and sign and date it as proof that you submitted it.
HEAT AND HOT WATER LOG If you do not receive regular heat and or hot water, not only should you call 311, you can use this form to document the time and temperature of the water/room for use when you go to court.
TENANT SURVEY ENGLISH/ENCUESTA DE INQUILLINO This is a general anonymous survey that REAC initiated for certain parts of the country.  Feel free to use it for your tenants association. 

COURT FEE WAIVER FORM when you are ready to take the landlord to court, use this form to document that you are low income and are requesting a waiver for the court processing fee
COURT INSPECTION REQUEST when you answer a court order or when you are taking the landlord to court, complete this form in order to have an inspector come to your apartment to verify necessary request for repairs
RENT DEPOSIT COMPLAINT If you have given a rent deposit and have not received the annual interest in check or a statement form, follow and complete the instructions on this form.

GUIDELINES FOR FOOD STAMP REPLACEMENT WHEN YOUR REFRIGERATOR BREAKS DOWN OR YOU SUFFER LOSS OF ELECTRICITY If you lost food because your refrigerator broke or you lost electricity at no fault of your own and you receive food stamps, read the guidelines in order to receive a full replacement of your food stamp budget.
